**Physical Therapist – Community Hospital of San Bernardino**

At Community Hospital of San Bernardino, we are looking for a licensed professional to provide high-quality care across a range of medical conditions and patient populations.

As a valued member of our therapy team, you will evaluate and treat patients using evidence-based practices, craft individualized treatment plans, and collaborate with interdisciplinary teams to deliver patient-centered care. This role allows you to make a meaningful difference while maintaining flexible scheduling.

**Key Responsibilities Include:**

+ Perform comprehensive evaluations and establish personalized treatment goals

+ Administer therapeutic interventions in compliance with physician orders and professional standards

+ Educate patients and caregivers on home exercise programs and recovery expectations

+ Document patient progress accurately and timely in accordance with hospital policies

+ Participate in discharge planning and coordinate with care teams to ensure optimal outcomes

+ Provide guidance and oversight to support personnel and therapy students

+ Contribute to quality improvement efforts and department initiatives

**Qualifications**

+ Current registration, certification, or license as a Registered Physical Therapist by the respective state of which one practices

+ BLS - Basic Life Support certification is required.

+ Bachelors (BA, BS) degree in Physical Therapy is required.

**Pay Range**

$75.53 - $75.53 /hour
