**Responsibilities**

**Position Summary:**

Assist individuals and communities to adopt healthy behaviors.

+ Conduct outreach for medical personnel or health organizations to implement programs in the community that promote, maintain, and improve individual and community health.

+ May provide information on available resources, provide social support and informal counseling, advocate for individuals and community health needs.

+ Provide services such as blood pressure screening.

+ May collect data to help identify community health needs.

**Qualifications**

**Minimum Qualifications:**

+ Experience working in the community.

+ High school diploma.

+ Will complete CHW or Promotoras training within a year of hire.

+ Ability to work independently, set priorities and manage work within timeframes.

+ Read and analyze technical material.

+ Communicate effectively verbally and in writing.

+ Positive mental attitude.

+ Strong customer service orientation.

+ Ability to handle highly confidential information essential.

+ Must be computer literate.

**Preferred Qualifications:**

+ Teaching experience.

+ Bachelor’s Degree.

+ Completed CHW or Promotoras training.

+ Bilingual (English/Spanish)

**Pay Range**

$17.12 - $23.54 /hour
